How to apply for hospice

  1. The basic conditions of a child's admission to the hospice:
    1. the child is diagnosed with an incurable disease and
    2. has been diagnosed and has finished normal tretmant and
    3. if he or she is over 14 years old his or her parents / legal guardians have agreed to halt the further diagnostic procedures and normal treatment and
    4. if he or she is over 14 years old his or her parents / legal guardians have agreed to start symptomatic treatment in children's home hospice and
    5. have been referred to the children's home hospice by the GP or the medical unit responsible for the treatment until now.
  2. To apply for the hospice care his or her parents/ legal guardians may:
    1. come to the hospice's office or
    2. make a phonecall
    3. write an e-mail.
  3. Hospice referrals are received by the Member of the Board responisble for medical care or during his absence by the Member of the Board responsible for nursing.
  4. We accept applications to the hospice:
    1. in the hospice's office: Monday to Friday from 9:00 a.m. to 3:30 p.m.
    2. on the phone: Monday to Saturday from 9:00 a.m. to 7 p.m..: +48 663 814 005 or +48 663 814 006

  5. Information about how to apply for hospice care is available to the interested:

  6. When we cannot admit a given child to the hospice, the parents / legal guardians may:
    1. put their child on a waiting list- every month we transfer data to Narodowy Fundusz Zdrowia: Malopolski Oddział Wojewódzki,
    2. sign their child up for a different medical unit of a similar profile close to the child's dwelling.
  7. After the child's admission to the hospice the Member of the Board responsible for medical care together with the Member of the Board responsible for nursing and the Coordinator of rehabilitation and psychologists establish the hospital unit providing the medical care and set up the date of the first appointment with the parents/legal guardians of the child.
